Dear Lori,
If youi are doing primarily an E&M type of intake with lots of medical comorbidities, medications, labs to order, records to review, collateral information to collect than use 99204 or 99205.  If it is more of a psychotherapy oriented intake with minimal medical aspects use the 90792, however it may be reimbursed lower than 99205 because they don't take into count complexity or medical decision making.

Am I correct in saying that if we use the 99204 or 205 we should also use the 90836 or 90838 with it?  Thank you, and thanks also for all of your diligent work on this as well as the fantabulous feedback!  Rhonda
