Per page 485 in the cpt code book, psychotherapy codes 90833, 90836, and 90838 can be used with patient codes 99201-99255, 99304-99337, 99341-99350. you cannot add a psychotherapy code onto a 90792. (page 484).

I commonly add on 90833 to 99213 or 99214. In our state UBH has let it be known that they expect the add on.

The claims I filed with Blue Cross with 90833 add on are coming back showing the patient needs to pay 2 co pays. I have contacted provider relations and been referred to claims processing. No response. My admin staff was already complaining bitterly about administrative burden with BCBSTX. We'll sees where this goes.
Best wishes,

It's really dismaying to see how obstinate the Blues have become with paying Psych E&M codes. The same thing here in CT.

In Texas we have "Delelegated Prescriptive Authority."  BCBS interprets this to mean that I can only be on their panel if my delegating physician is also on the panel.  My delegating physician has resigned from BCBS, therefore my problem is solved.  I was forced to resign for this reason.
Patients have out of network benefits.  They pay for services and submit their own claims.  This improves my quality of life significantly.  I will no longer have the blues.  My problem is solved.
No problem with United by the way.  Humana not so good.
