I also would recommend buying the CPT code book for 2013 there are examples and very clear discussion about these codes. I would be cautious about the add on codes. If you use them you must be sure your document supports therapy and or the complexity. Also for most insurances the higher code will pay more. For example a 99213 and and add on 30 min therapy will pay less then a 99214. The 99214 can be used if your document supports in HPI exam etc, low or moderate complexity but greater than 50% of the time was spent in supportive counseling. (30 minute appt) Time is only a factor when greater than 50% Is spent doing the counseling, A 99215 can work if you spend 45-60 minutes with document to support and greater than 50% of your time was spent in counseling. It is extremely rare that I do not do supportive counseling for more than at least 15 minutes with my patients. I think most of us do this so naturally, it is part of the art of nursing, that we forget we are doing it.
