I did one the other day and there are 6 lines for dates of service with each having one corresponding box for the CPT code. If we are using psychotherapy codes plus the E&M "add on" code that takes up basically 2 lines or 3 if using the complexity add on. That can mean only 2-3 visits can be submitted per form instead of 6. Anyone know if this form wil be revised or any thoughts about options? Mary, I noted you mentioned just using the 99 codes. So, for example if I am seeing a 22 year old Bipolar II client for therapy and complex medication management - I think I am hearing you say it is likely best i use the 99214 or 99215 rather than the therapy code plus the 99 code? Also glad you posted the initial eval code info ( 99205 ) to use vs the 90792. I have listened to all the webinars and printed out the transcripts for me and my students, printed out power points, crosswalks, forms, etc but still do not feel at all like I am sure of what I am doing. For example I would have totally used the 90792 until your post. I think this DB is a really important one right now. thanks!
