Here are some billing results  from UBH/HP = Optum:
99214 paid $25.50 + $20 copay = $45.50  Old 90862 and 90895 paid $50 including copay = now I am shorted $ 4.50 per bill

99215 paid $60.75 + 15 copay =$75.50
90807 paid $80 with copay so this is $4.50 short

So from what I can tell billing E/M codes which was suggested as useful when we do med education and counseling, etc., > 50 %, is not going to keep ups afloat financially

Billing 99214 = $ 25.50 + 20 copay + 90833 which paid $38.75 as an add on = $64.25, $ 18.75 higher than 99214.

 I was not planning on using a lot of add ons, hoping that I would not have to use it a lot since I think a lot of what I do is in the "coaching/counseling" category, but I think if I was an insurer looking at 90805 = Meds + therapy, than should I be always billing 99214 + 90833?  I am not trying to get a big raise here but I cannot keep going shorted $ 5 each visit.

Thoughts?
