1.  CPT codes How do u adjust your fee

Hi All
Sounds like we are all struggling with coding.  When sending in claim forms we have to put in our charges. How are NP's adjusting the fee they used to charge based on the old codes? When dealing with the insurance companies we have agreed upon rates but I always put in my real charge. I dont know how to break up my fee or what to put in the charges section of the claim forms. Whats everyone else doing?????

I always put my real fee. Not the contrived and demeaning insurance fee. I purchased icanotes its a program that help u write notes then codes it for you. I love it and i dont have to worry about not meeting criteria, under coding or over coding.
Price is fair!!

I have a bit of a different and probably obvious question for you folks who bill routinely - but I'm not sure! I do not take insurance but always offer to complete the HCFA form for patients to submit for out of network or to help meet deductible. What I am not sure about is where to put the EM and then the add on code There are currently 6 lines for visits, but if you use EM plus add on code for therapy do you just use two lines for each date thus can only put 3 visits per form? I am a small private practice and do these by hand not electronically. I looked at the new forms dated 2/12 that will start to be accepted in Jan 2014 but that was not a part that i could see had been revised. The revision looked like it just added a number of extra DX code spots. I was looking at this online and maybe I missed something (I was using a small screen and it was a bit blurred). Curious as to how you guys who bill for the EM plus therapy bare doing this on a very practical level. Thanks in advance.
