1.  Audit experience?

Thank you Mary and Kathleen,

I am reassured by your message, Mary. I use a SOAP note format, with a symptom checklist that I have patients fill out in the waiting room. I was given it by a psychiatrist I worked with when I started my private practice, and modified it only slightly to use with the new CPT codes. It works well for me--I use the O for my MSE as you do, and document the counseling under P. I looked carefully at the CPT definitions of counseling and care coordination before I decided to use this option, and I do feel that counseling and coordination of care is an accurate description of at least 50% of my appointments. But the hassle factor of having bills pended and records requested is enough to make me consider other options. I'd love to know more about your experience with an audit. It is nerve-wracking not knowing what is going to happen next!

I'd be very curious to learn more about your symptom checklist if you would be willing to share. Sounds interesting. The whole audit thing is not pleasant to say the least.
