Just wanted to know if there is any tool that has been developed to rate a patients Mental health symptoms according to the severity for a inpatient adult mental health unit.
For example if some is admitted for depression with suicidal ideations with no plan VS a patient who is admitted with depression who acted on with a laceration VS a patient who has paranoid thought process with increased aggression.
