Initial 24 Hour Assessment

Hello,

I am in need of advice on initial assessment of an acute inpatient psych patient.  When, how ofter etc.  Seems our nursing documentation is not thorough enough. 

Also, documentation before seclusion/restraint?  Does anyone have a form or advice on this.  We need to be more thorough with nursing documentation.  thank you!

The following hyper-link may prove to be very helpful for this area of inquirey regarding retraint and seclusion assessment. I would check with other inpatient psychiatric units in regards to restraint flow sheets.

In regards to your first question regarding initial assessment it may serve you well to first assess the patient with a follow up assessment by your collaborating physician which is usually done within 24 hours, of course you always have the option to consult him with any problems or questions as he must be available to you.
