Our facility which consists of adolescent, adult and geri utilize a self made satisfaction survey of 10 questions. We encourage the patient to complete on discharge and they are placed in a box and we review them monthly. Questions are related to helped by psychiatrist, nurses, social work, groups, safety, surroundings, etc.
