CSSR challanges

I work in a state of the art psychiatric facility.  We are finding that our CSSR screens are most commonly high (hence the need for psychiatric admission) and we are needing to place patients on higher levels of observation.  I don't understand the intent around the required screenings when we have patient in a psychiatric safe environment. This is creating huge staffing barriers.  As an example, last night we had 18 patients, 2 where CO and 5 where IO based on the CSSR scores. 

 Does anyone have any more detailed information around the new standards set by Joint Commission?  How are you dealing with the high scores?

We have a ‘high risk safety plan’ (a document) in our admission packet that assists with the high scores/staffing issue. If identified as high risk, (which so many do due to the questions) we ask that they identify items on the list that help them cope while inpatient, ie a weighted blanket, a stress relief ball, music, things like that. At the bottom of the form are three areas for them to initial and then a signature stating they feel they will be safe while in the hospital. So often they score high with absolutely no CURRENT SI. Of course sometimes they still require a higher level of staffing but this has helped us greatly.
